However, upon arrival, she discovered that both the telephone line and the intercom system were out of service. This communication breakdown prevented her from alerting staff in time to intervene. A jury ruled that Olugosi's death was a suicide.
The report also revealed that Cambridgeshire police and the Metropolitan police were unable to reach the prison by telephone, as calls were being diverted to an unmanned or obsolete number.
